Depending on what part of Africa you will need several shots in order to be immune from diseases. I would say a hep shot would do it. You will also need a passport and your license or state ID in order to get through customs in the US and when you reach Africa.

You will need a passport and a birth certificate. It's the same when you travel to any country but your own.

There are certain vaccinations you are required to get when traveling to Africa. You should look for them on government websites. You are also going to need a passport.
