This problem could be your voltage regulater, locate voltage regulator and test it. Some voltage regulators are built inside the alternator depending on year of car. If it is located inside you will need to replace the alternator.

Most likely if you are having problems with your alternator it will be that the coils inside of them have gone bad. You will need to buy a new one.

Many things. It cold be loose. The bolt on the bottom could also be loose. It could be old and worn. If new it could be for the wrong year.

Alternators are subject to wear like any part on a vehicle and are one of the most replaced items over the lifetime of a car. Generally the problem is simply that the parts inside the generator are no longer within the tolerances required to create a sufficient electric charge. It probably just needs to be replaced.

positive cable rubbing against the steering shaft. It could also be that belt being loose. If you do the trick of pulling the negative cable, you should do it with the heat fan on high and the headlights on. Once you remove it, rev the engine a bit as well.
