It may be the muscles around the wound trying to rebuild the area. Depending on where they cut, it may be tendons trying to knit back together, the body is trying to protect itself, it wants to protect it's weak spots. Ask the surgen for clarification if it is concerning.

A knot in an incision area is often no more than where the stitches are. If the knot is painfull or tender to the touch you should call your surgeon.

A knot in an incision area can be scar tissue, stitches that are not fully healed, or a small hernia at the site of the incision. Ask your doctor.

It can be caused by the area, preventing infection from entering the body and stopping bleeding from any severed vessels. Can also be a sign of hematoma, meaning that the vessels have not stopped bleeding at the site of incision. This will be accompanied by the dark red to purple brusing in the region also.
