Give both constructive criticism and positive reinforcement. Do not fault them with criticisms you cannot support with examples, and provide solutions for the future. Let them know what you expect.

You should prepare an 'appraisal sandwich'. Start off with some good feedback and observations about their performance. Follow this with some critical points and how they can improve their performance. Finish with some feedback to leave them feeling motivated about their job and the work they're doing.

You should monitor their continuous performance in terms of goals that were set for them in the beginning and also assess their tacit abilities like integrity, people friendliness, leadership, team working etc. After making a preliminary assessment on these factors, check with his peers and colleagues. Finally, have a free talk with him on his performance before the final reporting.

This first thing to keep in mind is to be completely honest. Give some constructive criticism, even if it's negative, and then add positive reinforcements.

1. Include Positive and negative behaviors, date each entry, write observations, not assumptions, keep out biased language, track trends, your assessment of the quality of an employee's work, instances of tardiness, work absences or extended breaks, disciplinary discussions and actions taken, details of significant personal interations with the employees. Don't include rumours or speculations of employee's life.

Be objective, rate the employee according to his inputs and effectiveness, and not according to his/her personal relation to you or the boss.
