This should be done every 6000 miles or so. Usually it is best to do it at every other oil change if you can. Take the front driver's tire and exchange it with the back passenger's tire. Then take the front passenger's tire and exchange it with the back driver's tire.

The most appropriate way to rotate them so that they have a longer use, is to rotate the front tires to the back and the back tires to the front. This provides the best chance at a long tire life, and less chance of a tire blowing while in use.

The best way it to cross the 2 rear tires to the front and the front straight back(ie. RR to LF, LR to RF,RF to RR, LF to LR) In the case you have directional tires, the should be moved straight forward and back. This should be done every 5000 to 6000 miles.

You cannot rotate a tire on a car, if the tread is worn on one side there is a fault with the suspension or the balance could be off. There might also be a fault with the steering, an old tire might be able to be kept as a spare till you get a new one.

Always follow the manufacturers directions on how to do a tire rotation. If it doesn't specify you can try doing the typically way. Take each tire and move it to the opposite corner. For instance teh drivers front will be moved to the passenger's rear corner. This doesn't apply to directional which should only be swapped front to back.
