It, of course, depends on what you have to say thank you for. If it was a dinner party or holiday at a friend's place, thank them for their friendship and tell them how much you appreciate them. Also, maybe refer to particular incidents that were enjoyable. If it was a present thank them for the good choice of present.

They can be very awkward, but the best thing is to keep them simple and short (that's why they are "notes" and not "letters"). Say "Thank you" and briefly mention what it is you are thanking them for (a gift, an act of service) and sign your name. That's it.
