How do you use javascript controls with a textbox to generate a form?

You will need to transform the php to a Html with the various codes to make a textbox form. If you already have html, then all you have to do is copy and past the code to the generator and form a textbox.

Unfortunately there is a cap on the limit of words to answer your question. Try searching online, there are many tutorials on how to do so, there even scripts which you can download and try yourself.
