Well, it depends what its for. But, generally you have to sign a consent form that shows that you are okay with disclosing medical information to the company. The form will contain information about the doctor, medical records so forth.

This isn't as hard as it sounds - you simply need to sign a waiver that says they can have your information. They do all the rest.
