You must take the document to a bank or other place where a notary works. Cost depends on where you go and what the document is for.

To get a document notarized you can go to a bank or credit union. Most will notarize for really cheap, prices range but usually no more than ten dollars. You will sign the document in front of the bank employee who will then put a seal and their signature on it.
