How do you make a request for pain and suffering?

Pain and suffering can be hard to get sometimes. But you have to add the amount you are requesting in the paperwork when you file in the court. But you may not get what you want or you may it just depends on how you present it.

Each state has different laws regarding "pain and suffering. " Assign a value to yours and ask to be compensated. The judge might ask how you arrived at the figure.

If a law suit has already been filed then you can ask the jury to also award pain and suffering. This amount would be in amount in the enlightened conscience of the jury.

You would have to talk to your lawyer and have him or her help you out on getting pain and suffering, the lawyer can help you out.

If you have established the liability issue, then you get to damages. You must have evidence of the pain you have suffered. Your own testimony is evidence. However, the type of claim you have may preclude damages for pain & suffering. For example, most states don't allow this in workers compensation claims.
