To create .pdf forms, you have to use Adobe Acrobat professional. You open up a new file, and can design it any way you want. Then you save it as a .pdf form, and you can send it to anybody and they can edit it as well.

Download Software from Adobe and you can make a .pdf files everytime when you want. also you can make a .pdf files on line. type '' make pdf files online '' on google and you will find some page where you can make a pdf file on line. :) :)
