A 401K will be with the financial management firm for whom the company set up the 401K. Look at old pay stubs for what firm did the 401K withdrawal.

I think you would have to know the brokerage firm that handles the 401 for the company. If you can find that out then you can contact them and they should still have records of the individuals among the business. Most brokerage firms maintain a database of records for their customers so this should not be too hard to do.

If you have documents from your old company, including the holder of that401K, then try contacting them, calling their 800 number (most have 800 numbers). If you have your account number, make sure to give that to them, it will be easier for them to transfer your funds.

Having a summary plan description or a past quarterly statement will help immensely. Try to contact the plan administrator. If you are unsuccessful, you will need to look up the company's bankruptcy case to see if the court assigned a trustee to your 401K plan.

The 401K should be with the trustee or managing financial organization; it was not owned by the out of business firm. If you do not have financial statements from the 401K custodian, check your old paycheck stubs for the company that took the 401K contributions; then contact them for your account information.

If there is still money in the accounts, then you have to find out what entity is holding the funds currently which should appear on your statement.
