In order to get a replacement card, you will have to have your patient number, social security number and some basic information to pove your identity. Call the 800 number provided on the website or on one of your medical statments and provide the information to the customer care rep.

It is important to notify Medicare immediately. Explain to them that your wallet was stolen, and that your Medicare card was inside of it. They will most likely ask you a few security questions to ensure that you are infact the policy holder. They will then send you a new card in the mail.

you can contact the office where you originally obtained medicare or you can call the medicare office and have a new one mailed to you. another option is to go to a medicare office and have a new one printed for you.
