Pharmacies are governed by the same rules that apply to any other medical provider, such as a doctor. In short, a pharmacy is prohibited from disclosing information about any client to anyone without permission. A pharmacy must ensure that people do not accidentally see what other people's prescriptions.

Pharmacies are required to keep all medical information on patients in confidence, including medical conditions, current medications and allergic reactions. They are compelled to hold in confidence all customers names, addresses and telephone numbers, as well.

I think confidentiality policies in pharmacies are the correct strength. They allow someone else to pick up your medicine if they know the information such as your name and birthday. This is sufficient since the chances of someone who is not authorized to randomly pick up the medicine. You can increase confidentiality by requiring the person to authorize over phone.

Pharmacies should not give any personal information to any 3rd party without your permission. This includes not telling what medications you are on and they really should not even say whether or not you are a customer.
