A hernia can be painful, particularly when it first occurs, or if it is strangled and the blood supply is cut off. There is also a condition called testicular torsion where the testicle spins within the scrotum, cutting off the blood supply and killing the testicle. If the pain is severe or continues, it can be a medical emergency.

This could be several different things but most common is a muscle spasm in the area. If not then there could be bladder or kidney infections or worse conditions that could cause it.
