Epididymitis is inflammation of the epididymis. The epididymis is a small, oblong body which rests upon and beside the posterior surface of the testes. It consists of the head and a long, convoluted tube. This can become inflamed, sometimes as the result of veneral diseases, TB, prostatitis and urethritis. It is treated with rest, support of the scrotum, and antibiotics.

this is caused by inflammation in the epididymus. this is found in the back of a testicle. this condition can indeed cause pain.
