When the central area of the disc ruptures and comes through the outer layers, called annulus fibrosus. If there is a nerve is pinched or compressed, it can cause pain and discomfort. The most common discs to suffer from prolapse and protrusion are known as the L4/5 and the L5/S1 discs.

This is a disc that ruptures toward the spinal canal. It's bulging to the right zone of the spinal canal.

All that really means is that you you have a disc that is sticking out of your back and it is off to the right side of your back when you are looking at it from behind you. All the rest of that just means the internal direction the dis is facing and what angle it is sitting at.
