What is an affidavit for reformation on a criminal?

I can't find anything in the law that defines this- based on your question I would assume it is a stipulation, like a "parole plan" that an offender has to adhere to stating how they will perform in future to be a "better person".

These are sometimes used by probation offices or parole offices when a convict is attempting to get a pardon or a restoration of civil rights.
