It means FDA Category C Drugs are possibly toxic and dangerous to pregnant women. Tramadol can cause harmful effects to an unborn baby, so it should never be used during pregnancy. It's also known as Ultram, and is a centrally-acting analgesic (pain reliever). The risk is very high for birth defects and health problems in pregnancy.
