If you can't afford to consult an attorney, PLEASE consult your Court Clerk- they will often gladly help you with paperwork or steer you to an advocate offered by the court. Your Dept of Human Services will often help you with this kind of thing, or help you find Legal Aid that you can afford.

If you are the parent, go to the court in the county where the kids reside. File a motion through the Clerk. You do not need an attorney; the Clerk's office will explain how to do this. You will then attend the motion and schedule a hearing.
