I know that a lot of shoe manufacturers make men's dress shoes with an elevated heel, although I can't recall any specifics. I would imagine that there would be a specialty women's heels shop that would have something that would fit the needs. Another option is buying them custom made online.

alot of specialty stores do sell heels for men. try typing that into google and see what it brings up. im sure you can buy them online.

Shoe sizes for women often times will come in a "wide" size. For example, if you wear a size eight, you can look for an "eight wide". You can also have custom shoes made to fit your size and style, but these can be very expensive. The best place to look for hard to find shoe sizes would be online.

Yes, you can likely find some high heels for men, especially in costume or fetish shops :) However there are probably special stores catering to men, or online.

Many shoe stores carry shoes in wide sizes. If you see a size labled "10W" that indicates that the shoes are a 10 wide. These shoes are for both men and women, and are perfect for a wider foot.
