Franklin Roosevelt was the only "4-term" president, although technically he was not in there for the full 4-terms. He died before finishing his 4th term.

Prior to the ratification of the Twenty Second Amendment (which mandated that at president could only serve for two terms at maximum), Franklin D. Roosevelt was the only president to serve four terms. Ulysees S. Grant and Theodore Roosevelt both made attempts at a third term but were unsuccessful.

There were no four-term US Presidents. Franklin Delano Roosevelt served three terms, not four, and no other President has served more than two. The constitution now limits the number of terms a President can serve to two terms, and therefore it's now impossible to serve more. Nobody besides FDR has ever served more than two.
