It depends where you lost your passport. If it is in a foreign country, then report that to the local police and your embassy on the same day. Check everywhere and everything as it is quite a pain to get a new passport issued. Ask around if people found a passport.

If you lose your passport at home report it missing immediately by filling out a form ds-64 and mail it to the u.S. department of state. if you are out of the country you should contact the nearest embassy or consulate immediately. the u.S. government offers a express to receive a new passport immediately. call npic to schedule an appointment.

Yes, you should report to the authority if your passport is missing because it might help you in the future if you want to replace your lost passport. Some authority sometimes ask for certain documents to prove that your reason is real and you are not making excuses.This thing would be a big help it the future.
