There is only one sure answer to severely rusted plumbing - - replacement. This is especially true if the piping is supply piping under pressure. It could spring a leak or burst at any time. Turn off the pressure, drain the system and start cutting. Remove pipe until you encounter good pipe threads. and re-plumb with new piping.

Since neither copper or PVC rust I can only assume you have some cast iron pipe that is showing some rust. Depending on how old it is it might not be a problem, they tend to go for a long time. Eventually, however, you will want to replace rusted pipe with PVC.

If you have rusted plumbing pipes, you have problems. Plumbing pipes should always be made of copper, which doesn't rust. Get a re-pipe, soon.

What you should do is use DLR cleaner and attempt to salvage them. If they appear to be beyong repair call a plumber to have them replaced.

You should call a plumber to inspect them. Rusted pipes can later burst or leak, leading to more costly repairs and damages. A plumber can make repairs to these pipes and get them working properly.

If they are leaking they need to be replaced. If not, I personally would not worry unless you believe the rust is inside also and in drinking water.
