Mildew stains can be very tough to remove. I would try applying a stain remover such as oxy clean or shout. If the fabric is white, use some clorox bleach. Allow the stain remover to penetrate the fabric and rub it in. Scrub gently with some laundery detergent than through it in the washing machine.

If the fabric is white or vinyl, then it is possible to remove the stains with chlorine bleach. Otherwise use an oxygen or enzyme cleaner. You may need to repeat the process.

Mix 1 cup Rubbing Alcohol with 1 cup water. Moisten a cloth with this solution and scrub mildew material. Place in the sun and allow item to dry completely. You could also mix lemon juice and salt. Spread onto item and leave in sun till it has dried. Rinse thoroughly and dry in the sun.

Bleach works very well to remove mildew stains but it cannot be used on cloth. To use bleach mix a bucket of warm water and bleach. Clean the mildewed area and rinse with clean water. If the mildew is on cloth use a mixture of warm water and Mr. Clean to clean and rinse with clean water.

You might need to just replace the cushions, or remove the fabric and sew on new fabric. Once fabric has those stains they are pretty much impossible to remove as far as I know.
