Bleach, cleaning solutions, and bacteria that can be found in a hot tub can irritate the skin upon contact. This contact irritation can cause inflammation of the hair follicles in the skin. This is called folliculitis.

The bacteria Pseudomonas aeruginosa thrives in warm, wet areas, causing an infection of the hair follicles when using a contaminated hot tub. The condition starts as a rash and then develops into tender nodules which may become pus-filled. To avoid permanent scarring, see a doctor for treatment.

It sounds like too much hot tub use but you need to contact the doctor that diagnosed you to be sure they will be able to break it down.
