What does it mean when my Toyota Sienna misfires?

This happens quite frequently.

This can be caused by several things that you can check. First check the sparkplugs and wires for excessive wear. If these check out fine check to the distributor cap to make sure it doesn't have a lot of corrosion on it. If that doesn't solve the problem its probably a fuel or air supply problem.

It means one or more spark plugs are bad. Replace the spark plugs and that should solve the problem. If not then change the coil packs.

You could have filled up with bad gas. If you are getting to higher mileage, you might need to replace your spark plugs which can be a common cause of mis-fires. You may also have a bad spark plug wire or a plugged fuel injector that is causing one of the cylinders to not work at 100%.

When was the last time it had a tune up? Old plugs and wires(if equiped) can casue misfiring as well as problems with the fuel system. If a cylider is getting to much fuel and is fouling the plug it will casue it to misfire. I would personally start with plugs.
