What does it mean if the vehicle title says "exempt" on the odometer reading?

Once a car meets certain requirements set by the DMV, like a certain age or being rebuilt, the odometer reading becomes unnecessary. So it is listed as n/a or exempted.

I usually means that either the odometer was replaced and didn't have the correct mileage or the cable broke and wasn't reading it
