There is a long standard form that you should use. The Nolo Press has great books, which include forms, to help landlords with such matters. I have personally used their forms and been quite happy with them. The advice they give, in addition to the forms, is invaluable. There are many legal pitfalls to avoid!

The court issues eviction notices, which states the date by which the tenant is to vacate the property. You will present evidence during the request for an eviction order.

You're going to have to file an answer to the notice and prove that it was wrongfully served either because the property is in disrepair or some other compelling reason.

you need to say how much back rent is owned. How long the have to pay it. Any other things that involve the person evicting.
