You can obtain a new certificate from your local court house. There is usually a nominal fee of less than five dollars for this.

What you will have to do is go to the records department in your city and request another copy of your marriage certificate. In most states this will cost you about $40. If you do it by mail there are normally some additional fees involved. Depending on your reasons for the name change you may not even need it.

Since I don't know where you live, this will have to be a general instruction of what to do. In my state, I would start by contacting the county recorder's office in the county where I was married. If that doesn't work, contact your state recorder's office.
