Excessive strain as a consequence of poorly fitting athletic wear is one cause. Manual manipulation of the testicles, often as part of bondage games, have been reported quite often in the emergency room setting. Testicular cord strains, all the way to full evulsion of the testicle have been described and certainly painful.
