This is called micturition syncope. Straining to pass urine causes vagal stimulation that drops the blood pressure and causes a blackout. Its better to sit when passing urine and not strain too hard. Rarely a phaeochromocytoma in the urinary bladder can cause this!

I had this happen recently. Blacked out, cracked my skull open and ended up in the hospital. After blood tests, ekg, ct scan, mri and carotid artery ultrasound, diagnosis was acute sinusitis. This surprised me as I have never had sinus problems in 60 years. Antibiotics fixed it.

If the blackout occurs right when you sit down, it might not have to do with urinating at all. You could have a chemical imbalance or vitamin deficiency. If it is just when you urinate, you could have some type of bladder infection.

You might be passing kidney stones and blacking out from the pain of it all. The other option is you might be that tired.

I'm no doctor but the only thing I can think of that causes that is vasovagal disease. It's very rare and doesn't cause alot of grief but it causes men to occasionally blackout while urinating due to a drop in blood pressure.
