You'll need to ensure that the engine will line up correctly to the transmission and that the mounts are correct for it. Kits should be available for that swap.

It depends on what model and year of truck you have as well as which hemi(old or new) you want to install. Swapping an original hemi into an older truck should be pretty simple where as a new hemi would be much harder. Look online for motor swapping article.

There are several things to take into account. First of all, motor mount will be different along with the location. Then changes will need to be made to the bell housing on the transmission. The fuel lines will need to be either lengthen or shortened and the frame cross member under oil pan may have to be relocated.

The difficulty will depends on which Hemi you want to install and what year truck you want to put it in. If you want an earlier model it will swap into a '61 to '71 Dodge Truck but may be harder if the truck is newer. Newer engines are smaller but require a lot of wiring and electronics to work.
