In Hawthorne's tale he probes the moral complexities of the psychology that surrounded the frenzy of the infamous Salem witch trials. It is filled with symbolic allegory and a introspective look at the Puritan lifestyle which seemed to add complex levels of psychosomatic hysteria, distrust and disillusionment that drove hundreds to darkness and despair.

I wouldnt say insanity is a main theme in the book, rather humanities struggle with the idea of evil and the way in which they believe it

The story explores issues of dealing with mental illness and the denial of mental illness. Keep in mind that the story was written long before our enlightenment about mental illness.
