Only a rapid streptococcal test of the area can confirm the diagnosis because symptoms are very similar to diaper rash or normal irritation. Look for perianal rash, itching, and pain.

Signs include rectal itching, pain, blood-streaked stools, and a moist, bright red perianal rash. Sometimes a mucous discharge is present as is a yellow crusty substance.

Your toddler would have a very red rash around the rectum. The toddler may exhibit need to scratch due to itch and pain in the rectum. If the toddler has received anti-fungal treatment and has failed, it may very well be that it is not yeast and actual strep infection.

Does the child have strep throat or just getting over it? With Rectal Strep the child might complain of rectal pain and itching and pain. Talk to your doctor.
