Obviously, the 21st birthday is significant because the person is now legal to drink. So, some type of reference to the now 21 year old celebrating this milestone birthday is require for the card. Something like "Happy 21st! Hope the headache tomorrow is worth the fun tonight!" would work because it references the drinking they'll undoubtedly be doing that night.
