Don't feel awkward. It's the thought that counts! You could say something like "I'm so sorry to hear you aren't feeling well. I am sending positive thoughts your way and hoping that really soon, you will have a better day. The point of this entire card is to say, Here's to wishing you a speedy recovery."

It depends on how well you know the individual. It's best to say, "I hope you're well soon." If getting well isn't in the cards for this person, then say, "I'm thinking of you" or "You're in my prayers." Briefer is better.
