Yes, an applicants past achievements are a great source of information. This information provides insight as to the type of work the applicant is used to performing as well as how confident they are in themselves.

Yes, you should. The past achievements are what shows that you did solid work, not merely shuffled papers or answered the phone. Leave off "employee of the week" and state "exceeded service level agreement by 5%".
