You start with a few cell (not necessarily stem cells) of the type of tissue you want to create, and place them with the appropriate nourishment in a Petri dish.

There are a variety of ways that scientists culture human tissue in a lab. One way is extracting healthy skin cells from a donor, and growing it on a protein matrix soaked in a pro-growing fluid. The fluid is rich in nutrients. Then, the scientists peel the artificial tissue off of the matrix. Voila.
