Pathological causes for a perforated bowel include Crohn's disease and diverticulitis. Crohn's disease is inflammation of many parts of the intestines, and diverticulitis is infection of small pouches in the wall of the colon called diverticulae. Trauma can also cause perforation. Compression from seat belts during car accidents or stab wounds are different kinds of trauma to the abdominal region.

Perforated bowel can occur for a number of reasons including from the rupture of an ulcer, as a result of diverticulitis, secondary to trauma, secondary to surgery, or as a result of chronic inflammation of the bowel (i.e. from ulcerative colitis or Crohn's disease).

A surprising number of things can cause a perforated bowl - the most obvious is trauma, but this is often not the case. It can also be caused by gastric ulcers, infections, appendicitis, SMA syndrome, diverticulitis (a certain kind of inflammation) or a foreign body (like fish bones)....

There are several ways that a bowel can become perforated. An ulcer can progress to a hole in the bowel. A foreign object ingested by mouth can puncture the bowel. Severe constipation can cause the colon to rupture. Cancerous tumors can reduce bowel integrity and lead to perforation. Also, structural aberrations like volvulus can lead to a painful perforation.
