How do you write a survey or questionnaire?

Create a list of questions to be asked. Make the answer selection easy, such as agree to disagree ranges in which they circle the answer or yes/no.

First thing to formulate in writing a survey or questionnaire is indentifying the topic, which should be spefic and aligned to your objectives. Most preferred surveys or questionnaires are those that have easy to follow instructions and should not take up more than 10 minutes of someone's time. Thus, a single page and multiple choice type is the ideal format.

Once you have the overall topic of your survey, start devising questions that you want to ask a respondant that corrolates to the topic. Then, decide if you want it to be a multiple choice question, or a free response. Often, surveys are comprised of both types of questions because it helps them learn the most.

First think of an interesting topic that you would want answers to. For example, what kind of ice cream do the students in my class like? Then you come up with sample answers like chocolate, vanilla, strawberry, or other. This is a way to create a simple survey or questionnaire.
