In general, "you" should not do anything unless "you" are emotionally stressing the child out as a result of your own frustration. While there are some physical reasons for this and you should DEFINITELY consult your pediatrician about testing for such conditions, many times it's an issue of growing out of the problem.

You would need to figure out what is happening to cause this to happen and then try to stem the tide of having whatever it is happen again.

The best way to modify behavior in children is positive rewards. Never scold the child for accidents, instead provide encouragement and praise. Also, giving small rewards like stickers help.

This depends on the age of the child and if there is any medical problems that make toilet training and avoiding accidents difficult. Setting times for regular bathroom visits can help children who are easily distracted by activities don't want to stop to go to the bathroom.

Take them to the bathroom immediately after wetting their pants. If they can't urinate, wait a few minutes and take them to the bathroom again. Patience is the key.
