I hope you mean, "How do you sew WITH a buttonhole attachment?". In which case there are several different kinds. It also depends on what kind your sewing machine takes. First off, you mark where you want the buttonhole(s). Then you fallow your sewing machines instructions for your buttonhole attachment. Some are special feet others are automatic.

A buttonhole attachment will have a space in which you insert a button, and that will tell the machine how large to make the button hole.

You'll need to measure and mark the hole so it's the size of the button. It should be more of a slit than a hole.
