First you will need to locate the hose. Then it is as simple as removing the clamps for the hose and replacing it with the proper part.

It depends on which vacuum hose you're asking about but, most vacuum hoses slid off without the use of clamps. Smaller, non-molded hose can be purchased by the foot at your local parts retailer. Molded hoses are dealer only.

The vehicle has a number of vacuum hoses or lines. After you find the correct/leaky one, it is usually a simple matter to just pull the hose off of the fitting with your fingers.

it depends on which vacuum hose you are talking about. generally they are very easy to replace and just take a few minutes. Usually they are held by two clamps one on each end. so a very easy job

It all depends on the vacuum hose you need to replace. Typically, you remove the hose from the vehicle, get a new one and simply swap them out. It's a piece of cake for most hoses.
