First you would figure out how deep you wanted the pole to go into the concrete. Your next step would be a design of the finished concrete you wanted the pole to be in. From there, it's as easy as mixing the concrete, setting the pole and putting up support.

The difficulty of this depends on the diameter of the pole. If its small use a concrete bit on a hammer drill. If its a 2" or more diameter pole, which is more likely the case, drill many holes in the area you want the pole, chip out the remaining concrete with hammer

You should install the pole before pouring the concrete. Drill down with the correct size bit you need to create the opening for your metal pole. Then, insert the pole in the ground.

It all depends on the size of your metal pole. The simplest way and answer to your problem would be to get a hammer drill with the correct size bit you need to make the hole for your metal pole. Simply drill the hole and insert your metal rod. Not much to it.
