Use Visual Basic editor in Excel to insert a userform to input and accept data. Save the code in a file and set a trigger using macro to execute this userform.

It sounds like you want to create a form to send to people which they can fill out, save and send back. The best way to do this is to lock the cells you dont want changed with a password. The other cells can be changed by anyone.

Creating a permanent input form in Excel is very easy. The easiest way is "to lock" all cells but the ones delegated to input than protect your worksheet and save the project. This way the user may only modify the blank fields you didnt lock, that's it: easy and effective!
