How do I transplant a blue juniper?

Just dig up a portion of the roots and transplant. Unlike flowers, juniper is a very tough and woody perrennial and will take to new ground quite easily. If unsure, just make multiple root cuttings to transplant.

I don't know how large your juniper is, but it's basic. Dig well around the plant, and well below, you don't want to cut into the root ball. Replant into a site with plentiful room and nice mulchy soil. A B-1 shock preventative can be used to avoid "shock"- ask your local garden center. This is a hardy plant.
