How do I set up a DBA for self-employment tax?

I do not see why you would need to set up a DBA ( doing business as ) for self employment purposes. Why not use your own name and social security number for tax purposes? That is what I have done in the past and never had any problems for tax purposes.

If you are not setting up an llc or corporation, add the name to all your business paperwork, including your checks and advertising. John Jones d/b/a John's company.

Set up an incorporated business with yourself as the owner. Then get a Taxpayer Identification Number or TIN for that business. If you simply want to be self employed for a business as yourself, you can get a checking account for "XXX - Doing Business As - Your Name".

You would do a Schedule C, for Sole Proprietors. Your net income from your business is subject to self-employement tax.

Usually you can go online and set the dba for self employment or you could go to the bank or elsewere and they do it
