Assuming a "frame-up", or 'complete', restoration; you would start by removing the engine and transmission, body panels, wiring and cab down to the frame. Then you would repair and paint the parts in reverse order. A machine shop can handle honing of the engine components to your specifications before reassembly. A body shop can handle a complete paint job.

Just restore it like any other vehicle. If you don't know where to start purchase one of the numerous restoration books. Just make sure parts are available to restore it.
