There are several online sites that can give you the wording of the document and/or offer a free template into which you simply plug in the names of the principals. Do a search for "wording power of attorney" without the quotes. You'll find lots of information

You can do this without an attorney, and there are many forms and instructions online to guide you. You need to check with the court to see how properly to file this, how it should be witnessed, how many copies, etc. This power can be abused, trustees are advised.

I'd strongly suggest you consult a local attorney who is familiar with the legal requirements of your state and will be able to prepare a document to meet your needs. You can usually buy generic forms at stationary stores or online, but they rarely will suffice.

You see a lawyer who can first prepare the proper paperwork and make sure that everything meet's legal requirements. Then it's best to have a witness to the event.
